Quick Answer

A senior living trust score is a transparent, data-driven rating that tells families whether a senior living community can be trusted with their loved one's care. The SeniorTrust™ Score from CareGrade™ aggregates regulatory inspections, staffing data, complaint histories, verified reviews, and digital transparency into a single 0–100 score with a letter grade (A–D). It is the only trust score system built entirely from verifiable government data.

What Is a Senior Living Trust Score?

When families search for senior living communities, they're bombarded with marketing claims: "award-winning care," "five-star service," "premier community." But none of these labels are backed by verifiable data. A senior living trust score cuts through the marketing noise.

The SeniorTrust™ Score is built on a simple principle: a community's trustworthiness should be measured by what regulators and inspectors have documented, not by what the community says about itself.

How the SeniorTrust™ Score Is Calculated

The SeniorTrust™ Score uses six weighted categories:

1. Care Quality — 30%

The largest weight. For SNFs: CMS health inspection ratings, complaint investigations, deficiency citations, abuse indicators, and Special Focus Facility status. For RCFEs: CCLD inspection records, Type A and Type B deficiency counts, and complaint histories.

2. Staffing & Engagement — 20%

CMS staffing hours per resident per day, turnover rates (RN, total nursing staff, administrator), and weekend RN coverage. For RCFEs: complaint patterns suggesting staffing issues.

3. Environment & Comfort — 15%

Inspection-reported environmental deficiencies, verified amenities, pet policies, and safety features.

4. Family Satisfaction — 15%

Google review volume, rating, recency, and content analysis for recurring themes and staff stability signals.

5. Value — 10%

Verified pricing data with confidence scores, compared against quality indicators.

6. Digital Transparency — 10%

Whether pricing is public, license status is verifiable, and inspection reports are linked.

What the Grades Mean

  • A Grade (85–100): Excellent regulatory records, strong staffing, clean inspections, verified data transparency.
  • B Grade (70–84): Solid choices with minor areas for improvement.
  • C Grade (55–69): Notable concerns — multiple deficiencies, low staffing, or complaint patterns.
  • D Grade (below 55): Significant regulatory or operational problems.
  • BV (Basic Verified): Licensed and operational but lacking sufficient data for a full score.

Why Trust Scores Matter More Than Ever

The Information Asymmetry Problem

Families choosing senior living face a massive information asymmetry. The community knows everything about its operations; the family knows only what they're shown on a tour. Trust scores close this gap.

The Referral Industry Conflict

The senior living referral industry generates billions by placing seniors in communities that pay referral fees. SeniorTrust™ Scores are calculated independently — a community that participates in CareGrade™'s referral program receives no score boost.

The Regulatory Data Advantage

Government inspection data is the most underutilized resource in senior living selection. CMS and CDSS publish detailed inspection reports, deficiency citations, and staffing hours — but this data is buried in government databases. The SeniorTrust™ Score surfaces and translates it.

  1. Start with the grade — filter to A and B grades in your area
  2. Read the score breakdown — understand which categories drove the score
  3. Cross-reference with inspection reports — linked from every community page
  4. Ask informed questions on tours — if staffing is low, ask about RN coverage
  5. Track scores over time — communities can improve or decline

Frequently Asked Questions

Can a community pay to improve its SeniorTrust™ Score?

No. The score is calculated entirely from government databases, verified reviews, and public inspection records. No financial relationship with CareGrade™ affects the score.

How often are trust scores updated?

Scores update when new regulatory data is published. CMS updates quarterly; CDSS updates vary by inspection cycle.

What if a community has no trust score?

Communities labeled "BV" are licensed and operational but lack sufficient public data for a full score.

Are trust scores better than Google reviews?

They serve different purposes. Google reviews capture subjective experiences; trust scores capture objective regulatory data. The SeniorTrust™ Score incorporates verified reviews as 15% of the total, weighted alongside regulatory data that Google reviews don't include.
