Veterans Benefits for Long-Term Care: A Complete Guide

Millions of American veterans are entitled to benefits that can significantly reduce the cost of assisted living, memory care, and nursing home care — and the vast majority of them never receive those benefits because they don't know they exist.

If you or a loved one served in the U.S. military, this guide is essential reading before making any senior care financial decisions.

The VA Aid & Attendance Benefit

The most significant and underutilized VA benefit for senior care is Aid & Attendance (A&A) — a pension enhancement that provides monthly cash payments to eligible veterans and their surviving spouses.

As of 2025, monthly benefit amounts are approximately:

  • Veteran alone: up to $2,300/month
  • Veteran with dependent spouse: up to $2,727/month
  • Surviving spouse of veteran: up to $1,478/month

These amounts are adjusted periodically for inflation.

Eligibility Requirements for Aid & Attendance

Military Service

  • At least 90 days of active duty
  • At least one day during a period of war (World War II, Korea, Vietnam, Gulf War, etc.)
  • Discharge under conditions other than dishonorable

Care Need — The veteran must need help with at least one of:

  • Activities of daily living (bathing, dressing, eating)
  • Being bedbound or in a nursing home
  • Having a disability requiring regular medical attention

Income and Net Worth

  • The VA assesses annual income against a Maximum Annual Pension Rate (MAPR)
  • Net worth limit (as of 2024): approximately $155,356
  • Unreimbursed medical expenses (including care costs) can be deducted from income, dramatically helping eligibility
  • A home is generally excluded from net worth calculations

The Housebound Benefit

For veterans who are substantially confined to their home due to permanent disability but don't require the level of assistance required for Aid & Attendance, the Housebound benefit provides additional monthly pension income. It cannot be combined with Aid & Attendance.

VA-Contracted Nursing Homes

The VA directly contracts with certain nursing homes for veterans who meet eligibility criteria. This is separate from Aid & Attendance and provides actual care placement (not just financial support). Availability is limited and determined by local VA Medical Centers.

State Veterans Homes

California operates several State Veterans Homes that provide nursing care, assisted living, and adult day health services to veterans and their spouses at significantly reduced rates. These facilities are licensed, inspected, and specifically designed for the veteran community.

California State Veterans Homes are located in:

  • Barstow
  • Chula Vista
  • Fresno
  • Lancaster
  • Redding
  • Yountville

Surviving Spouses: Don't Overlook Your Benefits

The surviving spouse of a veteran who served during a period of war may qualify for Aid & Attendance even if the veteran passed away years ago. This is one of the most commonly missed benefits in elder care planning. Eligibility requirements mirror those of veterans, with adjusted income and net worth thresholds.

How to Apply

Applications for VA pension benefits including Aid & Attendance are submitted to the VA directly, using VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid & Attendance) and VA Form 21P-527EZ (Application for Pension).

Important: The VA does not proactively notify veterans of benefit eligibility. You must apply. There are no retroactive payments to the date of eligibility — benefits begin from the date of application.

Consider working with:

  • VA-accredited attorneys or claims agents (many work on contingency)
  • Veterans Service Organizations (VSOs) such as the American Legion, VFW, or DAV — who provide free claims assistance
  • State veterans service agencies — California's Department of Veterans Affairs (CalVet) offers free assistance

Beware of unaccredited "VA benefits consultants" who charge upfront fees. This is illegal.
