Fall Prevention in Senior Living: A Family Guide

Falls are the leading cause of injury-related death among adults over 65 in the United States. A single fall can result in a hip fracture, brain injury, or a downward health spiral that permanently changes a loved one's trajectory. For families evaluating senior living options, fall prevention practices are one of the most important quality indicators to examine.

Why Falls Are So Dangerous in Older Adults

The statistics are sobering: one in four Americans over 65 falls each year. For older adults, the consequences are rarely minor. Hip fractures — the most common serious injury from falls — require surgery in most cases, and the one-year mortality rate after a hip fracture in older adults can be devastating depending on age and health status.

Beyond physical injury, falls often trigger a cascade: fear of falling leads to reduced activity, which leads to muscle weakness and balance problems, which leads to more falls. Breaking this cycle requires proactive, systematic prevention — not just luck.

The Two Categories of Fall Risk

Intrinsic (person-based) factors:

  • Muscle weakness, especially in the legs
  • Balance and gait problems
  • Vision impairment
  • Cognitive decline (increases fall risk 2–3x)
  • Medications that cause dizziness, sedation, or low blood pressure
  • Chronic conditions like Parkinson's, arthritis, or peripheral neuropathy

Extrinsic (environment-based) factors:

  • Poor lighting
  • Uneven flooring or area rugs
  • Bathrooms without grab bars
  • Furniture at inappropriate heights
  • Cluttered pathways
  • Inadequate footwear

The best fall prevention programs address both categories simultaneously.

What Good Communities Do

High-quality communities implement fall prevention as a systematic, ongoing process — not a one-time assessment. Look for:

On admission:

  • A comprehensive fall risk assessment using a validated tool
  • Medication review for fall-risk drugs (sedatives, blood pressure medications, etc.)
  • Environmental assessment of the resident's living space

Ongoing:

  • Regular reassessment after any fall or health change
  • Physical therapy involvement for balance and strength
  • Exercise programs specifically designed to reduce fall risk
  • Non-slip flooring, grab bars, appropriate lighting in all units and common areas

After a fall:

  • Post-fall analysis to identify cause and prevent recurrence
  • Family communication within a defined timeframe
  • Documentation in the medical record

California Regulatory Requirements

Under Title 22, California RCFEs are required to maintain a safe physical environment and to report certain serious injuries, including falls with significant injury. Skilled Nursing Facilities operating under CMS regulations face additional requirements including mandatory fall prevention protocols.

Questions to Ask When Touring

  • How do you assess each new resident's fall risk?
  • What fall prevention programs do you offer (physical therapy, exercise classes)?
  • Can you share your fall incident rate or how you track fall data?
  • What happens after a resident falls — how quickly is family notified?
  • Has this community had any inspection deficiencies related to falls or environmental safety?

How CareGrade™ Can Help

CareGrade™ profiles include CMS staffing and quality data for Skilled Nursing Facilities, as well as CCLD inspection history for assisted living communities in California — both of which can reveal patterns in fall-related safety performance. Our SeniorTrust™ Score factors in care quality and regulatory history so you can compare communities on dimensions that matter most to safety.
